PLEASE NOTE:  This is a closed event.  REGISTRATION AND PAYMENTS MUST BE RECEIVED BY THE DEADLINE.  NO ONE WILL BE ADMITTED WITHOUT PRIOR REGISTRATION.  Parents are responsible for the transportation of their child to and from the event.  If you have any questions, please call Alma Small or Anna Menchaca at 822-1605

Lock-In Rules and Release Form

 

BRUNSWICK THOUSAND OAKS BOWL WILL PROVIDE FULL STAFFING FOR THIS EVENT.  WE REQUEST THAT PARENTS REVIEW THE FOLLOWING RULES WITH THEIR CHILDREN.  THESE RULES WILL BE ENFORCED IN ORDER TO PROVIDE A SAFE, ENJOYABLE TIME FOR EVERYONE.

 

1.           Although the event begins at 12:00 pm the participants are welcome as early as 11:30 pm.

2.           No alcoholic beverages may be brought in or consumed by any participants.

3.           Smoking by any participant will not be allowed.

4.           No horseplay or roughhousing will be tolerated.

5.           Fighting will be cause for being sent home.

6.           All offices and phones, with the exception of the pay phones, will be off limits.

7.           Any part of the house past the foul lights is also off-limits.

8.           Any abuse or damage of house equipment will not be tolerated.

9.           Violation of any above rules will be cause for parents being called and asked to pick up their children, with no refund.

10.        Although fire codes prevent us from actually locking doors, security will be at the door and no participant will be allowed to leave between the hours of midnight and 6:30 am.

11.        Participants will be allowed to leave early, if their parents pick them up.  Legal guardians must provide proof of guardianship.

12.        All participants will be required to check in and bring a copy of the release form with them.  Spare copies will be provided during check in.  Anyone not having a signed release form WILL NOT BE ADMITTED.

13.        Everyone is encouraged to bring a pillow and blanket with him or her.  (Optional)

14.        Early registration and pre-payment are needed.  The event will be limited to the first 100 paid entries.  (There are 3 other parishes participating.)

15.        Chaperones are needed.  If any parents would like to assist as chaperones, please contact Alma Small at    822-1605.

16.        All lanes will be turned off at 6:00 am to allow for cleaning and maintenance.
